What is some information I should be requesting on the parent/student info sheets?

This is my first year teaching.What info would you suggest reqursting on the information card? I teach first grade.

I had to make out the sheets for the grade two teacher i helped out. On hers we put the...
-student's name
-birthday(so she could give them a treat on their birthdays)
-custody info if applicable (like does he/she lives with mom dad or guardian)
-contact numbers
-emergency contacts
-how they get home (bus or pick up)
-alternate arrangements for pick ups (such as they're mom picks them up all week except Tuesdays when the babysitter does)
-allergies
-medical conditions
-activities they enjoy (which can help you plan events and activities)

There was more but that's all i can remember
